Solterra will offer personal care services that are tailored to allow residents to live independently. All personal care plans are to meet the needs of the residents with the families and an interdisciplinary team.

Our services are built upon encouraging independence, well being and privacy of our residents. Each resident is given the opportunity to decorate their own home with their personal belongings and individuality.

Medication Management

• Coordination of medications between Personal Care physician and pharmacy
• Organization and allocation of medications
• Scheduled reminders to take medicine

Personal Assistance

• Personal hygiene assistance
• Regular wellness checks of apartment home
• Incontinence assistance
• Clothing selection and dressing assistance both morning and evening

Schedule/Transportation Coordination

• Scheduled transportation to medical appointments
• Planned activities and outings both within residence and outside of Solterra
• Grocery shopping and errands

Weekly Housekeeping/Laundry

• Personal laundry as needed
• Bed making and linen changing services
• General housekeeping services as needed (empty trash, freshen up bathroom, tidy kitchen)
